A “clinical chameleon” with more than 250 symptoms

Histamine sensitivity is often described in medical literature, for example in the journal Intestinal Research, as a "clinical chameleon".


Due to its heterogeneous nature and the complexity of the metabolic pathways involved, the imbalance of this biogenic amine is capable of mimicking dozens of other pathologies and leading people into an exhaustive cycle of underdiagnosis.

A multisystemic metabolic syndrome. 


The root of this diagnostic complexity lies in the distribution of histamine receptors. According to an extensive review published in the journal Pharmacological Reviews, histamine exerts its effects by binding to four specific receptors (H1 to H4), which are widely distributed throughout the human body [1] . When the organism presents a failure in the degradation of this molecule, generally due to a deficiency in the DAO or HNMT enzyme, histamine overflows into the bloodstream, simultaneously activating the gastrointestinal tract, the neurological system, the cardiovascular, the respiratory and the skin.


Clinical practice and specialised literature report that histamine overload can trigger more than 250 symptoms or associated conditions. Among the most documented clinical manifestations , the following stand out:


  • Gastrointestinal: visceral pain, severe abdominal distension, episodic diarrhoea and nausea.

  • Neurological and psychological: histamine acts as an excitatory neurotransmitter in the central nervous system. Its accumulation deregulates the circadian cycle, causing maintenance insomnia, mood changes, anxiety states, as well as severe migraines and brain fog.

  • Cardiovascular: postprandial tachycardia, palpitations and episodes of hypotension, resulting from the potent vasodilator effect of histamine.

  • Women’s health: there is a feedback loop between histamine and oestrogen. Excess histamine aggravates menstrual cramps (dysmenorrhoea) and is strongly associated with the mediation of neuropathic pain in endometriosis.


We estimate that an adult person can visit, on average, up to ten different specialists (from gastroenterologists to rheumatologists and psychiatrists) without obtaining conclusive answers for their clinical picture.


By understanding histamine sensitivity not as a sum of isolated complaints, but as a systemic metabolic syndrome, health professionals (especially nutritionists and dietitians) gain the necessary perspective to treat the root cause. Rebalancing requires the modulation of the histamine load through diet and the restoration of the intestinal barrier, thus silencing the receptors that activate this true “clinical chameleon” [2].
